We too have museum collections, archival collections and a library. The
managers of the museum and archival collections when aquiring new
collections (often mixed lots ie. Medals, uniforms, artwork, archival etc)
each fill out the necessary paperwork for their part of the collection,
which is then coordinated by our registrar who ensures that usually only one
gift agreement is sent out the donors and that there is one form that the
owners sign giving the museum ownership and rights to the objects in
questions.

Our Library operates with different forms and procedures, because books in a
library can be more fluid (you can often buy a new copy to replace a lost or
missing book). The library acquires books through donations and by purchase.
I believe that there is a module in the library database system which allows
them to document how the acquisition was made and from whom.

Subject: All purpose acquisition forms

I currently work for and organization with a museum, archives and library;
each
having their own acquisition forms.  Management is looking into creating an
all
purpose form for all three sections.  Does anybody work for an organization
that
currently uses a form like this and are there any drawbacks to the practice?
