Hi Carrie,
I guess I'm not clear on your definition of conservation.  We always tell
the public that we do not do conservation treatments on artifacts and
leave it to the professionals- the conservators.  A conservator should
provide you with a very detailed report including photographic
documentation of before and after treatment.  (If they don't, you should
find another conservator).  We simply file this report in the object's
accession file.  We supplement this by doing our own condition report
before it goes to the conservator for treatment and then update the report
after it has been treated.  We also file this condition report in the
object's accession file.
If you are talking about stabilization or minor cleaning or preventive
conservation (such as building a supportive storage mount or reframing a
print into an acid-free mat) that you do, I would suggest a before and
after condition report that is filed in the object's accession file.  I
hope that this helps and that I am understanding your question correctly.
