I posted a sample of a form we use for one of our summer school classes for kids on the musedocs yahoo group.  It includes language for permission to take promotional photographs.  There are many parents who do not want photographs of their kids used in any way, so your photographer must be deligent!

We also have a seperated form for photos/video interviews that I will try to add later, and are in the process of developing one for videos that may or may not be sold.
