Take a look at "Great Tours!" (AltaMira Press, 2002), which includes a
chapter on guide evaluations and has samples for each of the different types
of evaluation you mentioned. You can order it on-line either from AltaMira
Press or preservationbooks.org.

Hi All!
Currently, I am creating docent evaluations for our
institution. If anyone would be able to share theirs with me,
I would greatly appreciate it! We have a good-sized docent
corps, and I am considering a three-pronged approach: they
evaluate their training, they self-evaluate, and we evaluate
them.
