I have a related question--what if you have name such as "T. Smith?"   Is it
proper to write, "Dear Mr. or Ms. Smith?"  I have always opted for "Dear Sir
or Madam" in this circumstance because it is impossible to ascertain the
proper address from a first initial and last name.
