I offer the Reader's Digest  version to your question:
Definitions of:
Collection Policy........Why you collect (includes why not to collect and why to "deaccession") and for what purposes.  Legal and ethical considerations included.  Board approves this document
Collection Plan......... What are the collection's strengths and weaknesses and what specific artifacts/objects/specimens you want to collect and for what reasons or purposes (given the collection's strengths, weaknesses, and facility/staff/resources capablities).  Strategies and schedules (When to collect) included.  Senior management approves this document, keeping in mind that it should be done in concert with an Interpretation/Exhibition Plan......
Collection Manual......How you collect.  Approval flow charts and sample forms, etc. included.  Collections Manager approves this document.

Our facility, which employs two people and has had paid staff for less than
3 years, does not have a collections management plan. To date, our unwritten
collections management plan has been "Try to get through the massive
collections backlog we have already, try to prevent it from happening again,
try to get everything in the computer, and try to get the storage area to
look less like my parent's garage and more like a collections storage area."

Lately, as we make progress on some of these things, I find myself wondering
what an actual collections management plan looks like. We have a Collections
Policy, but not a management plan. What are the differences? And would
anyone be willing to send me copies of their plan?
