Since it hasn't been mentioned yet . . . The Walker Art Center in Minneapolis has done some wonderful work with teenagers through (at least in the beginning) a Lila Wallace-Reader's Digest Fund grant.  Projects include a Teen Arts Council to which an office, a budget and a staff advisor were committed.  Walker staff members have done presentations about this project at conferences, and could probably send you explanatory materials.
