The form for signing over a registered copyright is called  "Recordation of
Transfers and Other Documents"  This form is available online from the
copyright office in PDF format and is extremely simple and quick to fill
out.  If the item has not been registered with the copyright office and it
has not expired than you can simply register the item at the copyright
office again look online for the form.  When you are forming your gift
agreement you really do not have to do anything special with it to include
copyrights.  Just make  sure they are mentioned in the document.  "Donor
assigns any and all copyrights" or something like that is the safest,
easiest and clearest  way to do it.  As for tracking we are starting to keep
track of expiration/renewal dates and the actual copyright number.  The best
way is to work these numbers into your forms just like an accession number.
I would not try to track your collection with a copyright number since not
every item (only those with registered copyrights) have them and your
accession number will do the trick.

Does anyone have any examples of copyright templates or forms that they or
their organizations use? If so I would be interested in seeing any and all
variations on the theme. With proper acknowledgement of any copyrights, of
course!
